ONLINE SCREENING TOOL

Assists patients in their upcoming health check visit at TMC; this should be filled out 24 hours before the scheduled appointment

Provides personalized and technical assistance to address any possible difficulty an HMO member may experience; assists in scheduling appointments and LOA coordination with healthcare providers for onsite outpatient utilization of our services
